Payment Options
We accept Visa, MasterCard, American Express, and Discover, as well as cash, personal checks and CareCredit. If your visit is covered at our office by health insurance, we will file your insurance claim for you.
Insurances Accepted
Our doctors are medical providers for most area health plans. If we participate in your health plan, the cost of a visit may only be a co-pay. In addition, we accept assignment of medical benefits from Medicare, Anthem, Piedmont Community Health Plan, United Health Care, Humana, and Cigna. If you're covered by more than one insurance plan, please bring a membership card for each.
To reduce waiting during your first visit, you can download and fill out our Patient and Insurance Information form. Simply download, print and bring the completed form with you to your visit.
